Orleans Essex VNA & Hospice is honored and committed to be part of our patient’s healthcare team.  Our staff along with your primary care provider work together, in partnership with you, your family, or caregiver(s), to provide the highest quality care.  We encourage you to be an active partner in your care – it is the best way for you to help us take the best care of you.

Please read more about these services below.  If you feel you need services ask your primary care provider for a home health referral or call us (802) 334-5213 for one of our experienced nurses to review your needs.  Many insurances require certain conditions for home health services.  We can review these with you.

Skilled Nursing

From providing customized care to individuals with a chronic illness or condition, to providing post-operative short-term care, home health care services can help individuals remain independent at home.

When is home health care right for you or someone you love? Here are some signs that might indicate a need for in-home health care:

  • Change in ability to complete daily living activities
  • Change in appetite, weight loss or difficulty in swallowing
  • Dependence on a caregiver
  • Difficulty managing medications
  • New medical diagnosis and a need for teaching
  • Wound care needs
  • Difficulty with pain
  • Difficulty in breathing

Home health care services allow some people to come home from the hospital sooner, avoid stays in health care facilities, or learn to manage a newly diagnosed illness or disease.

Rehabilitation Services

When you or a loved one require rehabilitation at home, the Orleans Essex VNA & Hospice can provide a complete array of services to help— from physical therapy to speech therapy. If you or a loved one is planning a hip replacement, knee replacement or other joint replacement surgery, after-care is just as crucial as a successful surgery. Our experienced therapists bring the highest quality of care right in the comfort and convenience of your home.

Private Duty

Home Health Aides/Licensed Nursing Assistants

Our Home Health Aides are a vital link between the patient, family members and other healthcare providers. Because they play such an important role, we look for certain qualities— compassion, patience, reliability, and a true desire to help others.  Home Health Aides are trained, screened, insured, and work under the direct supervision of a registered nurse in order to assist you or your loved one with the activities of daily living.

Wellness Clinics/Support Groups

Foot and Nail Care Clinic – We offer Foot and Nail Care Clinics, at the Agency in Newport and some satellite facilities, with care provided by one of our nurses. 

First Year Firsts – Bereavement Support Group meets the first Tuesday of each month 3:00pm-4:30pm at the Agency. There is no cost and all are welcome.


“High tech nursing” is medically necessary care for individuals who are technology-dependent or individuals living with complex needs; exceeding the care provided by skilled nursing visits in frequency, duration, and complexity, outside the scope of practice of a home health aide or personal care attendant, and furnished to an individual in their home.

NOTE: All skilled services must have a physician’s, or their designee, referral for care. Many insurances, including Medicare, have a specific criteria that must be met for admission and ongoing.  Please contact your physician or our office for additional information.
